Medicare EHR Incentive Program
If you are participating in the Medicare EHR Incentive Program, your first step will be to register yourself through their website. You should do this as soon as possible.
In addition to selecting an Electronic Health Record for your practice or institution, here if a brief look at the things you need to complete the enrollment process:
- National Provider Identifier (NPI) for all eligible professionals, eligible hospitals, and critical access hospitals.
- Provider Enrollment, Chain and Ownership System (PECOS) enrollment record for all eligible hospitals and all Medicare eligible professionals.
- CMS Identity and Access Management (I&A) user ID and password. For eligible professionals, use your ID and password for the National Plan and Provider Enumeration System (NPPES) or request them through CMS.

Once you have registered with the program and are meaningfully using your EHR according to the requirements outlined by CMS, the next formal step of the process for the Medicare EHR Incentive Program will be to begin the attestation phase where you indicate how, when, and in what capacity you fulfilled each requirement.

Medicaid EHR Incentive Program
If you are participating in the Medicaid EHR Incentive Program, your process will depend on the state in which you reside and work. Additionally, you must meet the following qualifications in order to receive incentive payments:
- You must be an eligible provider: a physician, dentist, nurse practitioner, certified nurse midwife, or physician assistant (PA) practicing in a PA-led Federally Qualified Health Clinic or Rural Health Clinic.
- Your Medicaid patient volume must be at least 30% of your total patient volume. For pediatricians, the threshold is only 20%.
- Within a reasonable timeframe, you must implement certified EHR technology.
- Your use of EHR technology must eventually satisfy a number of “meaningful use” criteria.
